Common Red-Eared Slider Health Issues and What Causes Them

Most problems in captive sliders trace directly back to one of three husbandry failures: inadequate UVB, poor water quality, or an unbalanced diet.

Metabolic Bone Disease and Soft Shell

VCA Animal Hospitals describes metabolic bone disease in aquatic turtles as "an imbalance of calcium and phosphorus due to an improper diet, lack of adequate ultraviolet (UV) light, or a combination of both," showing up as a misshapen shell or deformed leg bones and, in severe cases, stunted growth that keeps a turtle from ever reaching normal adult size. Watch for a soft or misshapen shell, deformed leg bones, lethargy, swollen limbs, and difficulty swimming or walking.

This is the classic result of weak or absent UVB combined with a poor diet. Early cases can genuinely improve once lighting and diet are corrected, alongside veterinary care, but advanced cases can be permanent or fatal. A proper T5 HO UVB setup sized correctly for the enclosure is the single most preventable factor on this entire list.

Pyramiding Is Not the Same as MBD

These two get merged constantly, including in a lot of care writing, so it is worth separating them. Metabolic bone disease gives you a soft shell. Pyramiding gives you a hard one with raised, peaked scutes. The shell is growing abnormally in both cases, but not for the same reason and not with the same fix.

Pyramiding is widely assumed to be a calcium problem, and the one experiment that tested that directly found the opposite. Stancel, Dierenfeld and Schoknecht fed 48 young red-eared sliders four diets for 150 days, including calcium and phosphorus supplemented versions, and published the result in the title: supplementation decreased growth but did not induce pyramiding. Every turtle finished with a normally shaped shell.

Pyramiding is multifactorial. Tree of Life Exotic Pet Medical Center lists excess dietary protein, overfeeding and high-calorie diets, calcium and phosphorus imbalance, low humidity, no nighttime temperature drop, insufficient UVB, lack of exercise, rapid captive growth rates, and concurrent illness as contributors. Note where calcium sits in that list: present, but one item among nine, and the slider feeding trial above found supplementation alone did not produce it.

For an aquatic species the practical lever is usually growth rate rather than lighting. Overfeeding a slider, particularly on protein-rich pellets and meaty treats, grows the shell faster than it can form correctly.

None of this makes UVB optional. Weak or absent UVB genuinely causes MBD, which is the more dangerous of the two conditions and the one that turns up on this list far more often.

Shell Rot

VCA Animal Hospitals describes shell infections in turtles as "caused by bacteria, fungi, or parasites," often secondary to trauma, burns, or bites, and showing up as soft spots, pits, discoloration, a foul odor, or white and fuzzy patches, with ulceration reaching down to the bone signaling a severe case. Prolonged exposure to dirty water, an existing injury, poor basking access, or high humidity without a chance to fully dry are the usual causes. VCA Animal Hospitals warns that a shell fracture specifically "can become infected, or the blood supply permanently damaged, within hours of the traumatic event," which is why even a minor crack is worth addressing immediately rather than waiting to see if it heals on its own.

Mild cases often respond to improved water quality, dry-docking time, and topical treatment under guidance, but deep infections need veterinary intervention, debridement and antibiotics. Respiratory Infections

VCA Animal Hospitals notes that respiratory infections in turtles are "most often caused by bacteria and are often secondary to vitamin A deficiency" or poor water filtration, showing up as excess mucus bubbling from the mouth and nose, nasal discharge, lethargy, and wheezing, with a turtle tilting sideways while swimming if pneumonia sets in. Water or ambient temperatures running too cold, drafts, poor water quality, and vitamin A deficiency all contribute. This can progress quickly, treat it as urgent and see a reptile vet promptly rather than waiting to see if it resolves.

Aural (Ear) Abscesses

Visible swelling on the side of the head, along with difficulty eating or swimming, VCA Animal Hospitals describes it as a "large swelling on the side of the head, just behind the eyes." Often linked to vitamin A deficiency combined with chronically poor water quality or a bacterial infection. This almost always requires surgical drainage and antibiotics from a vet, it isn't something that resolves at home.

Vitamin A Deficiency

Usually tied to a poor diet, especially an all-pellet regimen without enough variety or fresh greens, VCA Animal Hospitals specifically flags iceberg lettuce, an all-meat diet, and poor-quality commercial food as the common culprits. This deficiency contributes to the eye swelling, respiratory problems, and ear abscesses covered above, it's a genuine root cause worth taking seriously rather than treating each symptom separately. A complete commercial pellet paired with actual fresh dark leafy greens, not pellets alone, is what closes this gap.

Emergencies vs. Manageable at Home

Difficulty breathing, severe lethargy, an inability to dive or swim properly, deep shell lesions, large abscesses, or a complete refusal to eat for more than a few days are all emergencies. Early soft shell, once UVB and calcium are corrected, mild superficial shell issues, and minor scrapes, once water quality is fixed, are generally manageable at home. The Husbandry Pattern Worth Remembering
